Fix issue when bootstrapping multiple maps, jquery's each was calling the callback with a parameter and it was attempting to use it as a settings array.

Fixed it by isolating the callback.

Rename permissions:
"show node map" -> "view node map"
"show user map" -> "view user map"
"user locations" -> "view user location details"

Remove 'set user location' permission, it's a leftover from the days that GMap used to be responsible for user locations.

Hopefully fix #371134: warning: Invalid argument supplied for foreach()...gmap_plugin_style_gmap.inc on line 172.

Also, I changed some % to %%. Although this wasn't a problem in practice, using a single % means relying on internal behavior of db_query.

Fix variable usage bug. It hasn't caused problems because the backwards compatibility variables are still being used as a fallback.
